Help With Forms

A guided session to help you feel more confident navigating ADHD-related paperwork. I sit with you and help you reflect on your genuine experiences so you can articulate them clearly and truthfully in your own words. We can work through assessment preparation questions, PIP form descriptors, or Right to Choose letters to your GP.

You must already have the forms in your possession before booking. This session is about guidance and support, not about me filling in forms on your behalf.

A few things to know

  • I’m here to support you, not do it for you. I’ll help you reflect on your genuine experiences and find the right words to describe them clearly and honestly. I won’t fill in forms on your behalf or write responses for you.
  • I’m not a professional advisor. I’m not a lawyer, doctor, benefits advisor, or welfare rights worker. Nothing we cover in this session counts as legal, medical, or benefits advice.
  • I can’t promise a specific outcome. Whether it’s a PIP claim, an assessment, or a diagnosis, the result depends on your individual circumstances and decisions made by other people. I can’t guarantee what happens next.
  • I’m not responsible for what happens after. I can’t accept liability for unsuccessful claims, assessment outcomes, or decisions made by third parties like the DWP, assessors, or GPs after you submit your forms.
  • Everything you write is yours. What you put in your forms and applications comes from your own truthful experiences, and you’re fully responsible for what you choose to write and submit.
